ABSTRACT

【Objective】 To investigate the efficacy of in vitro local high-frequency hyperthermia combined with drugs in the treatment of type Ⅲb chronic prostatitis (CP). 【Methods】 A total of 148 patients with type Ⅲb CP treated in our hospital during Jun.2020 and Jun.2022 were randomly divided into control group (n=74) and combination group (n=74). The control group received only drug treatment, while the combination group received extracorporeal local high-frequency hyperthermia treatment. The National Institutes of Health Chronic Prostatitis Symptom Index (NIH-CPSI), maximum urinary flow rate (MFR), average urinary flow rate (AFR) and International Index of Erectile Function-5 (IIEF-5) score were compared between the two groups. 【Results】 There were no significant differences between the two groups in terms of NIH-CPSI, MFR, AFR and IIEF-5 score before treatment(P>0.05). After treatment, these indexes increased in both groups (P<0.05), and the improvements in the combination group were more significant (P<0.05). 【Conclusion】 Medication combined with in vitro local high-frequency hyperthermia is effective in improving the clinical symptoms of type Ⅲb CP, which is worth clinical promotion.

ABSTRACT

Objective To investigate the effect and mechanism of extracorporeal shock wave therapy(ES-WT)on typeⅢB chronic prostatitis. Methods A total of 90 patients with CP/CPPS in our department from June 2015 to February 2017 were collected and randomly divided into experimental and control groups ,each with 45 cases. The control group was treated with hyperthermia. The experimental group was treated with ESWT. Both two groups were treated for 2 courses. Following parameters were collected after two groups were treated for 2 courses:chronic prostatitis symptom score (NIH-CPSI),the International Prostate Symptom Score (IPSS),the content changes in prostate fluid IL-8,TNF-alpha,IL-1 beta,lecithin and the content change of urine IL-8,TNF-alpha, IL-6. Results After 2 courses of treatment,compared with the control group,the overall efficiency of experimen-tal group was increased(91.11%vs. 66.67%,P<0.05). NIH-CPSI(13.12 ± 1.71 vs. 18.16 ± 1.33,P<0.05)and IPSS(8.03 ± 1.78 vs. 13.04 ± 1.89,P<0.05)scores were decreased. In terms of inflammatory,compared with the control group,in the experimental group,the content of lecithin body was significantly increased. The levels of inflammatory factors IL-8、TNF-α、IL-1βwere significantly decreased in the prostatic fluid and urine. Conclusion ESWT is safe and effective in the treatment of type ⅢB prostatitis ,which may be caused by reducing the local inflammatory response in the prostate.

ABSTRACT

Objective To investigate the clinical effect of primary internal fixation combined with flap transfer for Gustilo Ⅲ B open fracture of lower extremity with severe soft tissue defct.Methods From January,2008 to January,2013,15 patients with Gustilo Ⅲ B open fracture of lower extremity and severe soft tissue defect were treated with primary internal fixation combined with flap transfer.Among them,there were 2 cases of foot and ankle fracture,9 of lower-tibia and fibula,3 of upper-tibia and fibula and 1 of distal femur fracture.The areas of the flaps were 10 cm × 8 cm-28 cm × 16 cm.Three cases were treated by bone grafting because of bone defect.Results All patients were followed up for 6-24 months.All flaps were survived,and only small area of 2 cases with the distal necrosis were cured by changing the dressing; In 1 case,the wound was infection and healed after anti-infective and drainage treatment; 13 cases had bone union in primary stage,and the other 2 cases were achieved delayed union; the average healing time was 7.2 months,and no case of osteomyelitis was noted; the time of wound healing was 12-36 days,with an average of 18.1 days.According to lower extremity function evaluation form,the excellent and good rate was 86.6%.Conclusion It is a safe and effective strategy to treat the Gustilo Ⅲ B open fractures of lower extremity and severe soft tissue defect with primary internal fixation combined with flap transfer,which has advantages of reliable fixation,covering the wounds with satisfaction,recovering exercise early,shorten the treatment period,alleviating the suffering of patients and so on.

ABSTRACT

ObjectiveTo observed the effects of auricular point sticking auxiliary treatment on ⅢB chronic prostatitis.Methods20 patients with ⅢB chronic prostatitis were assigned to auricular point sticking combined with KeDuo-Hua therapy (trial group) or KeDuo-Hua therapy alone (control group).In 1 month of treatment cycles,the improvement of NIH-CPSI score was observed.ResultsThe pain or discomfort symptoms,voiding symptoms,quality of life score were (14.65±2.06),(5.80± 1.06),(8.05± 1.23) in the trial group respectively,and (14.10±2.05),(5.65± 1.31),(7.45±1.50) in the control group respectively before treatment; these values turned to (5.80±0.83),(2.80±1.15),(3.75±0.76) in the trial group,and (8.90±1.10),(2.90±1.37),(4.20±1.77) in the control group respectively after the treatment.The difference between the two groups was significant (P<0.05).ConclusionAuricular point sticking auxiliary treatment of ⅢB prostatitis is effective.

ABSTRACT

Objective To seek an effective method for the treatment of grade ⅢB tibial fractures.Method From Oct. 1985 to Oct.1999, 26 cases of grade ⅢB tibial fractures were treated by early soft tissue coverage(including 12 cases of free muscle flaps and 14 cases of local muscle flaps), early fracture fixation(including 21 cases of external fixation and 5 cases of non-reamed intramedullary nail) and early bone grafting(15 cases).Results The follow up duration were from 10 to 32 months with an average of 15.5 months. The success rates of free and local muscle flaps were respectively 91.7 %(11/12) and 92.9 %(13/14)?Infection occurred in 5 cases (3 cases of local superficial infection and 2 cases of osteomyelitis). The time for bone union were from 17 to 66 weeks with an average of 42 weeks. Conclusion "San-Zao"therapy,especially early soft tissue coverage and early fracture fixation is on effective management for grade ⅢB tibial fractures with the advantages of promoting wound healing and bone union and decreasing the possibility of infection.
